Your Name for Numerology Readings
This statement applies for virtually all personal numerology readings: Provide the name on your birth certificate.
In the USA, and probably in most countries, the birth certificate reflects the first official recording of the baby's name — the given name and the surname together recorded as the baby's name.
It is your full birth name.
The full birth name is the name to use for most numerology readings (exceptions noted later in this article).
Some countries or societies have a different protocol. The given name may be recorded but not the surname. (The surname may be known as the "family" name or the "last" name.)
Pythagorean numerology is the numerology practiced at this website. For personal numerology chart readings, the full birth name is the given name followed by the surname.
Whether a free personal numerology reading or a purchased full, in-depth, personal numerology reading, the reading generally is more accurate when the full birth name is provided.
First and Middle Names
Some numerology forms ask for your first name. There may also be numerology forms that ask for your middle name(s). Here are descriptions of what those are.
The full birth name is the given name appended with the surname.
The given name may have more than one name, in which case the first name of the given name is termed the "first name" and the "middle name" is the second and any additional names of the given name. (When there is more than one name as the middle name, it optionally may be referred to as the plural "middle names".)
Here are examples:

John Jones
John is the first name. Jones is the surname.

John Paul Jones

"Full Birth Name" Exceptions
There are exceptions to the convention that the full birth name should be provided. Here is a list.

Special personal readings that require only the first name, only the surname, only the initials, or a name other than your own.

Readings that do not require your personal name at all, such as some email address, telephone number, or company name readings.

Special-name readings such as readings specifically for married names or for common, private, familiar, or nick names.

For babies with the first recorded name being "baby girl" or "baby boy", the first official name that was assigned afterward (perhaps when adopted or with some other event) is considered the full birth name.

For name changes, get a reading for both the full birth name and for the new name. Both will have aspects of the whole. The more a new name is taken for granted (which may be months or even years after the name change), the more real the new name reading tends to be.
Non-English names should be calculated according to the non-English spelling of the full birth name. A new name that is assumed in order to better fit into an adopted country is handled as a name change would be.
